Depending on the nature of the claim you will need to print out and complete the appropriate form/forms below to submit with the supporting documentation from your medical providers. Most of the forms include an outline of instructions for completion.
Wellness benefits do not require a form and can be faxed to WGA Benefit Consultants at 405-513-8501. Please make sure that your provider documents include your name. It’s helpful but not critical if you can provide your employer name and policy number on the cover sheet.
